Hmmm, I'm guessing you haven't played point-based character generation RPGs such as GURPS. If I've got 100 points to make a character I can take a disadvantage (limitation, flaw) to get some more points. One option is to have a weaker character following me around that I have to take care of, a dependent NPC. Someone useful most of the time would give me 15 points, someone not helpful but mostly self-sufficient would be 30, and having to carry an infant as I'm adventuring would be worth 100.
So I'm claiming Kaylee is actually an NPC Mal's player bought so he could have more points for skills, and the occasional hostage situation is the price of that.
